I acknowledge that if my child/children fail to follow safety instructions or behave in a manner that endangers themselves or others, The Open Church reserves the right to remove them from the activity or gathering, where we will engage the parent/guardian to for assistance.

I, understand that participation in children's church events/gatherings includes a variety of activities that may involve potential risks, including but not limited to:

GAMES, CRAFT, SINGING, DANCING, PLAY BASED ACTIVITIES, PEER INTERACTION

I release The Open Church, its staff, volunteers, and affiliates from any and all claims, demands, damages, or liabilities arising from or related to my child’s participation in Children’s church events/gatherings.
